Step by Step Enrollment Procedures

If you plan to enroll your child for the next academic year, starting in August, please contact the school for pre-enrolment before August.  The hours for summer registration during the months of June and July are:

Tuesdays, 12:00pm-3:00pm & Thursdays, 8:00am-12:00pm

If your child is starting during the school year, please contact the school for an appointment.


Here are the steps to enroll your child:

1.     Bring documents

   a.     Proof of residence (e.g., gas, electric or water bill, signed lease; – phone bills are not accepted)

   b.     proof of age (birth certificate)

   c.      social security card or waiver

   d.     photo ID for you, and proof of custody if you are not the birth parent

2.     Bring medical forms (from your child’s health care provider)

    a.     Proof of immunization (Form 3231) or waiver

    b.     Eye and Ear screening form, Dental screening form (Form 3300)

3.     Complete these forms

     a.     Student Verification Form (only the white fields, grey fields are not needed)

     b.     Student Health Form

     c.      Student Rights and Responsibilities and Permissions Form   

     d.     Parental Interview/Photo Release Form 

     e.     Transportation Form

     f.       Home Language Survey (available at school in multiple languages)

4.     Review the options for lunch payments if you do not plan to send lunch from home.

     a.     Get ready to send money on Mondays in a marked envelope or sign up for prepayment at

                                                          i.  You need your child’s student ID–ask the front office staff to provide it for you as soon as it is available.

     b.     If needed, complete a free/reduced lunch form.

                                                         i.   You need to apply every year

                                                        ii.   Families who receive SNAP (aka Food Stamps) or TANF assistance can qualify. Families who receive WIC or have low income may also qualify

                                                       iii.   One application per family covers all children

                                                       iv.   Forms are available in English, Spanish and 31 other languages

                                                        v.   It takes about 10 days until processed, please make sure to send lunch money during that time

5.     Get the supply list for your child’s class here:

6.     To help with getting settled:

   a.     Ask your child’s teacher to put you in touch with the room parent for your child’s class

   b.     Review the Student Handbook here:

   c.      The PTA Welcome Committee is here to help new parents – leave your contact information with the front office staff and they will get in touch with you, or email them at

   d.     Consider joining the PTA (Parent Teacher Association)
